In the fast-paced world of high achievers, it's not uncommon to encounter multiple physical issues that hinder peak performance and overall well-being. Today, a fascinating case made me reflect on my role in a new light, akin to the characters in HBO’s Westworld who repair and reprogram their fellow hosts.

Patients often present with more than one problem, and it’s essential to identify the most pressing issue to address first. Today, I saw a lovely lady with what appeared to be a De Quervain’s syndrome mixed with some CMC joint arthritis. On the other hand, she had trigger fingers. The challenge lies in accurately diagnosing these conditions and planning the most effective treatment strategy.
Comprehensive Diagnosis and Targeted Treatment
In such cases, it’s crucial to perform thorough diagnostic work to confirm the conditions accurately. For my patient, this involved reviewing the symptoms, conducting physical exams, and planning the necessary imaging or tests. Once diagnosed, I considered the best approach for treatment. For her trigger fingers and the other issues, targeted orthobiologic injections could provide relief and improve function. I marvel at the physiological beauty and function of the finger tendon apparatus.
This process reminded me of Maeve from Westworld, who, despite being a host robot, becomes adept at repairing and reprogramming other robots. In many ways, my role is similar—I see myself as a sentient biohacker, a biological android repairing my fellow humans. This perspective brings new meaning to my work, highlighting the blend of art and science in the field of pain management.
