A focused examination of strength, sensation, reflexes, motion, and stability

Review of MRI and X-rays in the context of the clinical examination

Possible contributions from discs, facet joints, sacroiliac joints, nerves, ligaments, and stabilizing muscles

What prior treatments did, or did not, clarify about the source of pain

Whether urgent surgical findings or progressive neurologic deficits are present

Movement, strength, load management, posture, and stabilization may be addressed through an individualized rehabilitation plan.

Diagnostic & Image-Guided Care

When appropriate, targeted diagnostic or therapeutic procedures may help clarify and address a specific pain generator.

Regenerative Options

Selected autologous orthobiologic procedures may be considered when the diagnosis, anatomy, and treatment target support their use.

No nonsurgical procedure is appropriate for every patient, and regenerative procedures should not be represented as reliably reversing advanced structural disease. Individual outcomes vary.

What Does Treatment Cost, and Does Insurance Cover It?

Treatment plans are individualized. Cost depends on which structures are involved and what the evaluation shows, so we do not publish fixed prices. You will receive a cost estimate before any procedure is scheduled. These procedures are generally not covered by insurance. If you have Regenexx benefits, they may be covered. The evaluation is billed separately.

Nothing is booked until you have reviewed the estimate. Coverage, when it exists, depends on your plan. The staff can help you ask the right benefits questions. They cannot promise that a given plan will pay.

WHEN SURGERY MAY BE APPROPRIATE

When Is Back Surgery the Better Choice?

Surgery may be the more predictable option when neurologic deficits are progressing, the spine is unstable, or symptoms remain disabling after reasonable nonsurgical care. Emergencies such as cauda equina syndrome, infection, fracture, or tumor require prompt surgical evaluation. A second opinion should create clarity. It should not postpone necessary surgery.

A responsible evaluation has to be willing to recommend an operation. Fusion, decompression, or another procedure may be the sound next step for some people. The work of this visit is to match the plan to the problem, not to delay care that should not wait.

SIGNS THAT REQUIRE PROMPT CARE

Surgery may be the more appropriate option when there is progressive weakness, loss of bowel or bladder control, cauda equina syndrome, spinal instability, fracture, infection, tumor, severe neurologic compression, or disabling symptoms that remain unacceptable despite reasonable nonsurgical care.

The purpose of a second opinion is clarity - not delaying necessary surgery.

Fusion, decompression, or another procedure may be the sound next step for some people.

Meet Deborah Westergaard, M.D.

 

Deborah Westergaard, MD completed an ACGME-accredited Pain Medicine fellowship at Texas Tech University and is double board-certified in Anesthesiology and Pain Medicine. She has more than 35 years of experience in anesthesiology and image-guided procedures, with nearly three decades focused on chronic spine and joint pain. When nonsurgical treatment is appropriate, she performs these procedures herself.

She earned her medical degree from LSU School of Medicine and completed anesthesiology training at UT Southwestern Medical Center. Her role is to explain the findings, define realistic expectations, and identify which patients are reasonable candidates for nonsurgical spine care and which are better served by surgical consultation.

Frequently Asked Questions About Back Surgery Alternatives

Can back surgery always be avoided? No. Some conditions require surgical evaluation, particularly progressive neurologic loss, instability, serious compression, infection, fracture, tumor, or an emergency such as cauda equina syndrome. Other patients may have reasonable nonsurgical options after a complete evaluation.

Does a disc bulge mean I need surgery? Not automatically. The importance of a disc bulge depends on whether its location and severity match the symptoms and examination. Imaging should be interpreted in clinical context.

What are alternatives to lumbar fusion? Depending on the diagnosis, alternatives may include targeted rehabilitation, medication management, image-guided procedures, or selected orthobiologic approaches. These options do not substitute for surgery when instability or significant neurologic compromise makes an operation appropriate.

Can regenerative procedures repair a severely damaged spine? No treatment should be promised to regenerate an entire severely degenerated spine. Selected autologous procedures may be considered for particular structures and carefully selected patients, but candidacy and results vary.

Is a second opinion reasonable before back surgery? Yes, when time permits. A second opinion can help confirm the diagnosis, determine whether the proposed surgery matches the pain generator, and clarify reasonable alternatives. Urgent neurologic or surgical conditions should not be delayed.
